We Evolve Along With You.
We want you to live as independently as possible for as long as possible. But health situations can arise when you don’t expect them, so your best strategy is to be prepared long before it happens.
Imagine your spouse falls and needs support while they recover—you can get skilled help within your home base. What if you have planned surgery and need assistance with daily tasks like bathing and grooming? Planning for the “what ifs” in life brings you peace of mind, which is exactly why we built Life Plan Communities such as this.
As an Independent Living resident, you’ll have priority access to Assisted Living, Memory Support, Short-Term Rehabilitation and Long-Term Nursing Care, and other special benefits non-Trinity Terrace residents don’t have access to.
That’s what we mean by a full continuum of living—however your needs change over time, we have the tools to help.
Assisted Living
Treasure your independence in an Assisted Living apartment. You’ll enjoy a spacious one-bedroom apartment with a full kitchen, bolstered by the support of a designated caregiver. If you need help with daily activities such as bathing, grooming, dressing, and dining, someone is there by your side.
Memory Care
Cognitive health is one of the cornerstones of a joyful and meaningful life. If you and your loved ones are dealing with Alzheimer’s or another memory-impairing disease, our specialized Memory Support Program has specially trained staff, adaptive programs, and purposeful physical space. We treat everyone as an individual, and we are dedicated to preserving dignity and adapting to unique needs.
Skilled Nursing & Rehabilitation
Recovering from an illness or injury, or need longer-term nursing care? We are prepared for any eventuality with our on-site, Medicare-certified Skilled Nursing and Rehabilitation Center. You can expect a fully personalized Plan of Care in partnership with our staff of licensed and certified professionals. They work in collaboration with your private physician who you can continue to see on your own. Enhanced services include visiting doctors, wound care physicians, dentists, and podiatrists who can speed your recovery or help you manage your new normal.
Your best advocates
A long-term care ombudsman helps residents of a nursing facility and residents of an assisted living facility resolve complaints. Help provided by an ombudsman is confidential and free of charge. To speak with an ombudsman, call the toll-free number below.
